The following updates were made to the Contract Request form in Contracts +:
- If you are using federal contract (not grant) funds to pay for services, you must attach the federal funding contract (the awarding agreement, prime contract, or subcontract) to your contract request.
- Purchasing Services and the Office of University Counsel need this contract to determine required flowdown clauses to add to your services agreement.
- A new Risk Questionnaire question was added: “Will student financial information be disclosed to or created, hosted, maintained or accessed by the Provider?”
- You should answer “yes” to this question if student financial information will be shared with or accessed by the supplier. A “yes” response will trigger Gramm-Leach-Bliley Act (GLBA) language to be added to the contract.
As a reminder, the short form services agreement cannot be used if student financial information will be shared with or accessed by a supplier.
